There was a time when much of dentistry avoided “TMD” because of the controversies from camp to camp. Of course we all see patients whose joint function is less than ideal but no treatment is required. At the same time, how can we recognize how the dysfunction got that way, and why does it progress? And when will it progress? And how can general dentists consider these factors without changing their entire treatment philosophy and joining a new “camp?”
What an amazing discovery to learn that some of what we have been taught in dentistry is in fact dental myth passed down from generation to generation without science. Much has been made of “evidenced based dentistry,” and those of us who actually treat patients on a daily basis know that no practice can be successful by completely depending on evidenced based concepts alone. Many of the studies are flawed; and much of the science is contradictory. Nevertheless, there is a flow of information that follows the preponderance of the evidence and allows that science to guide the “art” of diagnosis and treatment decision making.
While many camps disagree on major issues of occlusion and joint position, it is fascinating to realize how well dentistry does despite these differences. To understand how they can all claim “success,” our goal has been to explore what these various philosophies have in common.
A model has emerged that helps us to understand occlusion and when occlusion matters. What we have is a model that helps to explain the wide range of symptoms, consequently helpings us to understand why so much of the dentistry we do is indeed successful. Our goal was to reduce the stress while helping patients who have non odontogentic problems. At the same time, we wanted to reduce the stress while dealing with patients who have true dental issues. Learning the restorative/pain disconnect frees the practitioner to understand the forces that threaten our patients and the dentistry we want to do for them. Understanding the model allows us to help more patients than ever imagined in ways that coincide with the terms of our learning curve. The model emphasizes diagnostically driven therapy – specific therapy based on an understanding of the craniomandibular system and the weak links that tend to be affected by excessive forces.
Dr. Barry Glassman Seminars offers courses for the general dentist who has an interest in basic concepts, as well as for the practitioner who wishes to enhance skills in diagnosing and treating joint dysfunction, headache therapy and pain management. Following the basic courses, general dentists can readily implement these basic concepts into their own practice without major conceptual shifts in treatment planning and restorative therapy. The Dental Sleep Medicine course is designed to be an all inclusive experience which prepares attendees, upon return to their respective offices to immediately begin treatment of patients with sleep disorders. The 2012 courses have been announced and will be in Allentown and include the potential to attend a viewing day before the course at the Allentown Pain Center.
There is our yearly October Annual Dental Medicine Conference which is specifically designed for the dentist actively treating obstructive sleep disorders. This two day meeting changes every year and provides not only an excellent review of dental sleep medicine but the latest information in the form of literature reviews and presentations from dentists and physicians alike. The literature review will keep you posted as to the latest updates in sleep medicine; as well as some of the most current information on oral appliance therapy. There is a full two hour presentation on reading signals in a PSG, and a section from Dr. Glassman reviewing cases that involved untoward complications. This is the opportunity for any of the participants to present a case for discussion as well. The physicians Dr. Glassman works with here in the Lehigh Valley present relevant specific topics in the field on the second day.
New to the schedule is the addition of an Advanced Dental Sleep Medicine course. This two day course is specifically designed for those who are actively treating patients. These include a refresher on the course content, case presentations, and individual guidance to help you in your treatment of patients with sleep disordered breathing. There is special concentration on bruxism, as well as complications of oral appliances and their treatment. An entire section is devoted to practice management dealing with growing the practice management as well.
New and exciting is the announcement of the in-house option. You can now schedule a private three or four day course at the Allentown Center for you and up to two staff members. If your main interest is in dental sleep medicine, Dr. Glassman will arrange a night at a local sleep lab for you. This course provides personalized training for you, your assistant, and your front office personnel. This course is designed to be useful for both preparation of the board exam for the AADSM and for general knowledge and increased confidence levels. These are intense training days for you and your staff.
The Traumatic Occlusion and Bruxism course is the basic course recommended for general dentists. This course is designed to change the way you think about occlusion and helps us understand why so many with “malocclusions” seem to function very well without pain! This course has the potential to change your life and the lives of many of your patients. Learn how to control the forces that threaten your patient and the dentistry you do for them!
NTI Hands-On Rehearsal ( schedule is pending) is a half day lecture and hands on program designed for you and your staff. This course will prepare the general dentist for NTI construction in their office or through Keller Labs. The details of the “whys” and “hows” will be taught, followed by a hands-on portion where attendees are guided through chair side insertions. An opportunity to purchase NTI’s at significantly reduced rates will be offered at prices available only at this course.
The In-House Pain course includes three days or four days of actual patient care that actually starts on the Sunday before the week begins. This is an intense course that is designed for either you or you and up to two staff members. You will observe full days of patient care in sleep, headache therapy, chronic pain pattern treatment, and joint dysfunction.
All of the courses given at the Allentown Pain Center also provide an option to observe a full day before the scheduled course, with the exception, being, of course, the In-House Pain Course.
What is unique about this course is that is now being scheduled privately and being personalized to your needs. You can schedule this alone or with one other office. You can come by yourself or bring one or two staff members.

This is a special event for those who are members of the on line forum. This conference has no set fee with costs being shared by participants as we plan to meet and share information for the two day conference. Are you a member? Call Angela for information.
The Allentown Pain Center treats patients with craniomandibular disorders, chronic pain, and obstructive sleep disorders. Patients are referred by all medical specialists, as well as by general dentists. Careful patient diagnosis occurs within a model that includes an initial phone contact, a comprehensive history, joint vibration analysis, indicated quality imaging, and a comprehensive examination. Non-surgical, diagnostically driven treatment is the key to the patient care employed at the Center.
Based on the patient's symptoms, the Center incorporates both permissive and demand appliance therapy. The key role of parafunction is emphasized. Supportive therapy, including physical therapy modalities, pulsed radiofrequency and injection therapy are prescribed as necessary. Dr. Glassman employs outcome measurement techniques and focuses on logical therapy and recall systems.
Conservative therapy with prudent use of Phase II therapy when required is emphasized.
